Intent to receive COVID-19 Vaccine
We are getting closer to the time when COVID-19 vaccine will be available for everyone; VCSU will receive vaccine for employees and registered students.
Your response to this survey is crucial to ensure that VCSU receives the right amount of vaccine. PLEASE complete this survey even if you submitted your response to the initial survey in January; it is vital to have accurate numbers of those already vaccinated, those intending to get vaccinated and those who decline to get vaccinated. If you do not complete the survey, it will be assumed that you choose not to vaccinate, therefore vaccine may not be available for you at a VCSU immunization event.
The survey will take less than a minute to complete and will close on Wednesday, March 24th at 11:59 p.m.
If you can get vaccinated before VCSU receives vaccine, please do so!
